A capacity assessment is a formal process used to determine whether an adult can make personal or financial decisions independently. It is commonly required for guardianship, trusteeship, supported decision-making agreements, or co-decision-making arrangements under Alberta's Adult Guardianship and Trusteeship Act (AGTA).
A capacity assessment can be requested by:
Family members or caregivers concerned about a loved one’s decision-making ability
Legal representatives or lawyers
Physicians, healthcare providers, or social workers
The Office of the Public Guardian and Trustee (OPGT)
Individuals going through a court process for guardianship or trusteeship
All assessments are performed by Mukesh Mishra, MSW, RCSW, a licensed Designated Capacity Assessor (DCA) in Alberta. Mukesh is authorized by the Minister of Seniors, Community and Social Services to conduct capacity assessments under AGTA.
We provide mobile services across Edmonton and surrounding areas (within 1–2 hours driving distance). Assessments can be conducted at:
Your home
Hospital or care facility
Assisted living or nursing home
Office or neutral location (if preferred)
We serve the Greater Edmonton Area, including St. Albert, Sherwood Park, Fort Saskatchewan, Leduc, Spruce Grove, Beaumont, and Stony Plain. We also travel to nearby communities such as Camrose, Wetaskiwin, Drayton Valley, Westlock, Barrhead, Tofield, and many others within a 1–2 hour radius of Edmonton.
Most assessments take 2-3 hrs, depending on the complexity of the case and the individual’s health or cognitive condition. Additional time may be needed for reviewing documentation or follow-up.
After the assessment, you will receive a Capacity Assessment Report, which includes findings and recommendations. This report can be submitted to the Office of the Public Guardian and Trustee or used in court proceedings, depending on your situation.

Yes. All information shared during the assessment is kept strictly confidential and is handled in accordance with Alberta’s privacy laws and ethical standards for social work and healthcare professionals.
You may be asked to provide:
A government-issued photo ID for the individual being assessed
Medical records or psychological evaluations (if available)
A court file number (if involved in legal proceedings)
Any Power of Attorney, Personal Directive, or previous assessments (if applicable)
