Talk it through
Call Mukesh. You explain the situation, and you find out if an assessment is the right step, what it costs and what to prepare.
See the doctor
Alberta’s process asks that a doctor first rule out temporary causes, such as an infection or a medication problem.
The assessment visit
Mukesh meets with the adult privately, at home or at the office, in the language they are most comfortable using.
Your report
Mukesh completes the Capacity Assessment Report (Form 4) and explains the result and your next steps.

People think most clearly where they feel safe. For an older adult, or someone with a disability, travelling to an unfamiliar office can make an assessment harder and less fair. Mukesh does in-home capacity assessments in private homes, seniors’ residences, hospitals and care facilities across Greater Edmonton.

About Mukesh Mishra
Mukesh is a Registered Clinical Social Worker with a Master of Social Work, and a designated capacity assessor under Alberta’s Adult Guardianship and Trusteeship Act. The office is in south Edmonton, and every assessment is done by Mukesh personally.

Common questions
How much does a capacity assessment cost?
Alberta sets maximum fees: up to $500 for a co-decision-making, guardianship or trusteeship assessment, and up to $700 for a combined guardianship and trusteeship assessment. You will get the exact cost, including any travel, before you book.
Can someone be assessed without agreeing?
An assessment should only happen for a legitimate reason, and the adult is told what it is about. Mukesh will talk with you about how to approach this if your family member is worried or unwilling.
Does a poor decision mean someone lacks capacity?
No. Adults are allowed to make choices their family disagrees with. An assessment looks at whether the person can understand the decision and its consequences.
